Thank you for considering Vails Family Practice as your healthcare provider. We are currently accepting new patients. If you are interested in becoming a new patient, please print and complete the registration forms below and bring your current insurance card and immunization records for all children to our New Patient drop in hours daily from 4 p.m. to 6 p.m. Plan to spend 10 to 30 minutes.

All new patients over the age of 18 are required to watch our New Patient Orientation Video which reviews what makes our office unique and what you need to know to ensure your experience is extraordinary.  You may watch this 20 minute video after registration or before your first appointment.

We hope that after your visit you will feel confident that you’ve made a wise decision by choosing our practice for your healthcare needs.
