Thank you for considering Vails Family Practice as your healthcare provider. We are currently accepting new patients. If you are interested in becoming a new patient, please print and complete the registration forms below and bring your current insurance card and immunization records for all children to our New Patient drop in hours daily from 4 p.m. to 6 p.m. Plan to spend 10 to 30 minutes.
We will then schedule you for an upcoming orientation where you will become familiar with how we work to best serve you, meet our providers & staff and tour our new adult & pediatric suites.
We hope that after your visit you will feel confident that you’ve made a wise decision by choosing our practice for your healthcare needs.
